My husband has suffered with eczema since he was a baby but is now told it is atopic dermatitis and he has severe sweating to the point he changes his tops multiple times a day and uses towels and kitchen towel next to his skin to soak up the excess. He's getting to the end of his tether with it and the specialists don't really know what to suggest. He's on methotrexate at the moment but that doesn't seem to be helping. The only things that brings any relief is steroids and he's had his quota because of the side effects. He moisturises all the time and does use steroid cream on his hands but his life has been put on hold as he's so bad. Any suggestions welcome please.

I am so sorry to hear about your husband suffering! It sounds like he needs the next line ot treatment. He should ask his dermatologist to try a biologic injection Dupilumab.

Dear Jackie

Other options to consider for treating eczema would be different systemic medications to replace the methotrexate (if its not really working). These include Azathioprine or Mycophenolate Moefetil.

Phototherapy (UVB) can also sometimes help.

For the sweating (i'm assuming this is a long standing problem and other causes such as thyroid dysfunction have been ruled out) you could consider an anti-cholinergic tablet such as Oxybutynin or propatheline bromide. These will however dry out the skin and the eczema may get worse, If it is localised sweating in armpits or groin then Botox injections will help, but this often is not available on the NHS.

All these treatments will need to be discussed with your Dermatologists, and I realize they may have been proposed already. Beyond this if your husband's eczema is bad enough and he meets the various criteria - Dupilimab could be considered.
