Hello! I have a problem with frequent pain and pressure in the bladder, day time frequency, with no urgency, no incontinence. I have had a few episodes in the past but after surgery in August last year (open uterosacropexy and pelvic floor repairs) it’s been bothersome most of the time. A cystoscopy during surgery showed haemorrhagic bladder. Recovery from surgery was long and painful due to complications. A second cystoscopy was normal. The cystocele returned after only 6 weeks; my consultant confirmed that and also “urethral descent”. Although first diagnosed as OAB, I discussed with my consultant that I think it’s more likely painful bladder syndrome. He now suggests trying ranitidine OTC but I’m not sure what dose to use, or how long to take. Can you advise please? Any other suggestions will be welcome.

I’m using Ovestin; can’t tolerate tolterodine, betmiga is ineffective. Otherwise- not overweight, fit and well at 61, I’ve cut out obvious bladder irritants and tried bladder training, meditation...all of that!

Thanks for posting such detailed info.
Your symptoms do sound like painful bladder syndrome (or true interstitial cystitis as you stated you had a haemorrhagic bladder).
This is a very debilitating condition as you mention but there are very specific treatment regiemes for this type of condition, ranging from antihistamine tablets, to bladder installations of medicine aimed at calming your symptoms. You should talk to your GP about a referral to a Dr who specialises in painful bladder with a view to staring to get treatment and feel better. Assuming the consultant you’re under isn’t one already, in which case I’m sure you’re in good hands. In the meantime, I know you’ve mentioned it but look at food triggers, cut out caffeine, alcohol, citrus and spices and be sure to check you’re not harbouring a low lying urinary tract infection. Keep fluids up at around 1.5L day.

i agree this is quite a difficult one and doesn't sound like simple OAB
You are also doing all the right things
Most likely a painful bladder syndrome/IC situation
I would get a referral to a specialist functional urologist. In London Guys has 2 and UCH 2 or 3.
You should be re evaluated with urodynamics as things have changed after your recent surgery.
Might be worth doing a general anaesthetic cystoscopy and bladder biopsy to prove IC
Bladder instillations may help such as Cystistat
