In this situation I often try and insert a larger than usual SPC - say 18 or even 20 French. The other issue is who is changing the SPC - it maybe better to have the changes in hospital if you are not already.

Recurrent cystitis or UTI is certainly something that you should seek help to control. It can be a huge interference with your quality of life and there are several potentially successful treatments available.

It sounds like your bladder is overactive - during periods where your conscious level drops - eg overnight/afternoon sleep this is resulting in urinary leakage - there are medications that may be able to help with that. I would advise completing a bladder diary in order to investigate whether you are making too much urine at night. This is a condition called nocturnal polyuria and the treatment is different to that given for overactive bladder syndrome - this is the first step in trying to find out what the problem...

I think its a difficult issue - persuading patients to seek help for urinary incontinence is a bit of a taboo subject.
