Almost always best to do a trans perineal biopsy
this can now be done under local anaesthetic in many centres

James Sorry to hear this. Unfortunately often there isn't much that be done and all pairs of testicles are slightly unequal in terms of size. If it is a new thing then worth checking. It could be due to previous infection, mumps, one not dropping until after the other as a child, previous trauma/her...

Simon you should get it checked please
Painful lumps are usually not cancer and much more likely to be infection or inflammation.
But still a small chance it might be significant.
You need an ultrasound scan of the scrotum to check things

Bohemicus Pieter has given you all very sensible advice. i would add that Underactive bladder is usually a diagnosis made after urodynamic studies and you should have these if you haven't. You might be helped by an alpha blocker type medication. Unfortunately there isn't a bladder power UP pill but ...

Richard interesting one Certainly enlargement doesn't come and go, but inflammation of the prostate can do. This might explain occasional flare ups. Could also be caused by infections. Our dietician may be able to help you more specifically but a diet high in fruit and veg is sensible. particularly tomatoes which have good levels of lycopenes in.
Reduce red meat and drink plenty of fluid.
you should have a PSA test if over 45 and then an MRI if indicated before any biopsy

Norman If you have had 2 biopsies previously then i wouldn't have another at this stage. An MRI would be a very sensible thing to do if they can manage it.
